How do I create a subscription form for my list?

To create a form, please do the following:

1. Go to the Contacts | Your Lists tab.

2. Choose the list that you would like to create the form for and click on Manage List to get to its List Snapshot page.

3. Click on the Subscription Form tab.

4. You will see that the Name and Email Address fields have already been created for your form.

5. To add more fields for your form, click on “Add Field”.

6. Name your “New Field”.

7. From the drop-down menu, choose the column you would like to map your new field to.  For example, if have you named your field “Telephone Number”, you should map this field to “Office number”, “Home number” or “Mobile number”.

8. If you would like to rearrange your form, you can click on a field row, drag it up or down and release it at the row that you want the field to be in.  At the same time, your form preview will be updated, showing you the latest sequence of your fields.

9. If you would like to remove any of the additional fields that you have created, simply click on the X button (this is the Remove button), followed by the Remove This Field link.

10. Next, tell us where to locate your form’s Success Page and Error Page.  We provide you with default pages (you can view them by clicking on the links provided), but if you would like to use your own pages, you will have to enter the URLs in the fields provided.

11. When you are ready, you can click on the Save Form button, which will bring you to a page where you can copy the form code.
    
    

We provide you with two ways to put your form on your website:

1. Put the form code on your own webpage

   1. Copy the form code and paste it onto your webpage.  If you are not familiar with this, please contact us and we can assist you with this for a nominal fee.

   2. Now you are ready to accept subscribers for your email campaign.

2. Host the form on our system

   1. If you would like us to host your form on our system, you can simply copy the link provided under “Host the form on our system” and put it on your webpage.

   2. When a user clicks on the link, he/she will be brought to your form, which is hosted on our system.

   3. Now you are ready to accept subscribers for your email campaign.
